This copy is preserved in the Beyazid Public Library with the number 1598, and it is written in Naskh script on 89 pages, with each page containing 25 lines. It is also void of the name of the copyist and the date of copying, except that at the end there is a record of ownership dated in the year 791. It is therefore one of the copies of the eighth century and was written before the mentioned date.
فِيInصَفْحَةِthe pageالْعُنْوَانِof the titleكُتِبَwas writtenاسْمُthe nameالْكِتَابِof the book:كِتَابُBookالْدَّاءِof Illnessوَالْدَّوَاءِand Medicine.,,وَاسْمُand the nameالْمُؤَلِّفِof the author,,وَفِيهَاand it containsعِدَّةَseveralقُيُودٍrecordsتَمَلُّكٍof ownershipوَمُطَالَعَةٍand readingوَفِيAnd inأَعْلَاهَاthe top of itعِبَارَةٌthere is a phraseضُرِبَتْstruckعَلَيْهَاupon itحَتَّىso thatلَاnotتُقْرَأَread,,وَنَحْوُهَاand similar to itفِيinحَاشِيَّتِهَاits marginالْيُمْنَىright.
In the title page, the name of the book "Book of Illness and Medicine" and the name of the author are written, and it contains several records of ownership and reading; at the top of it, there is a phrase struck upon it so that it cannot be read, and similar phrases are in its right margin.
وَفِيAnd inأَسْفَلِthe bottomالصَّفْحَةِof the pageخَتْمٌa sealيَحْمِلُbearingالْعِبَارَةَthe phraseالَّاتِيthe following:::وَقْفُEndowment ofهَذَاthisالْكِتَابِbookآغَةٌAghaالْمَشْهُورُfamousبِإِنْسَانٍfor being a humanزَادَZada...وَهَذَاAnd thisالْخَتْمُsealنَفْسُهُitselfتَرَاهُyou seeفِيinآخِرِthe endالنُّسْخَةِof the copy,,وَفِيand inأَثْنَائِهَاits midst(ق(Fol.٤٨/أ)48/a) also.
And at the bottom of the page, there is a seal bearing the following phrase: "Endowment of this book of the famous Agha known as Zada." This seal itself you see at the end of the copy, and in its midst (Fol. 48/a) also.

As for the records of ownership and reading, they are: 1 - From the ownership of the poor humble Othman Mir in the treasury of the year 1166. And beside it is a small seal that reads his name: Othman. This is at the top of the page, then another record was written under the title of the book in the left margin which reads: "From the grace of God Most High, the owner of this fine book, Abdullah Othman Mir, the weak, forgiven by God for his hidden sins, and concealed his faults with the Muslims. And awaken him (so) from the sleep of negligence...".
